Job Description

POSITION TITLE: Registered Nurse - Hemodialysis

REPORTING RELATIONSHIPS:

Report to the Supervising Nurse and the Alternate Supervising Nurse. Oversees licensed vocational nurse as necessary.

QUALIFICATIONS:

Ø RN with valid licensure through the Texas Board of Nurse Examiners

Ø At least I year dialysis experience work in an acute setting; at least 2 years dialysis experience to work in a home setting preferred, a minimum of 1 year required.

Ø Valid Texas driver license in the state with proof of valid auto insurance

Ø Ability to meet minimum Agency hiring standards

Ø Must provide evidence of current cardiopulmonary resuscitation (CPR) certificate

Ø Must be able to provide proof of TB and Hepatitis results

Ø Read, speak, write and understand English.

 

ESSENTIAL PHYSICAL DEMANDS AND WORK ENVIRONMENT:

Day to day work includes hemodialysis machine set up and therapy performance, personal computer work and interaction with patients, facility staff and physicians. Requires travel between various locations by automobile. Requires walking, standing, stooping and bending. Must be able to do lifting of at least 20 pounds. Work environment is characteristic of a home with air temperature control and moderate noise levels. May be exposed to infectious and contagious disease/materials.

 

POLICY:

Participates as an active member of facility staff and participates as needed with QAPI Committee.

Appropriate and timely communication must be maintained with the Medical Physicians, Supervising Nurse and alternate supervising nurse.

 

JOB SUMMARY:

1. Plans and provides professional nursing care for patients under the direction and in cooperation with the Supervising Nurse and in accordance with Simply Dialysis policies, procedures, guidelines, OSHA, TDSHS, TDADS, AAMI, federal, state and local regulations.

  1. Assists and supervises assigned staff members in the delivery of patient care.
  2. Performs patient assessments, obtains vital signs and additional assessment data, evaluates data and intervenes as appropriate.
  3. Initiates and terminates dialysis treatments on patients with permanent fistulas, grafts or catheters.
  4. Discusses changes in patient status with physician.
  5. Implements patient care plans.
  6. Administers medications and intravenous solutions as ordered.
  7. Maintains patient home medical record and reports.
  8. Submits patient record information into facility as required to maintain record completeness.
  9. Provides patient education and documents.
  10. Participates and directs emergency care as needed to the patient.
  11. Utilizes the nursing process to make recommendations for patient care.
  12. Evaluates delivery of patient care and communicates to appropriate personnel relevant facts about the patient, action taken and patient responses.
  13. Documents, verifies and ensures implementation of physician orders
  14. Participates in the delivery of patient care
  15. Participates in the team work of the company
  16. Observes activities and makes recommendations to supervisor for improvement in the quality and effectiveness of patient care.
  17. Implements and maintains compliance with Simply Dialysis policies and procedures.
  18. Maintains standards of cleanliness and organization of dialysis supplies and equipment in assigned patient locations.
  19. Works in collaboration with the interdisciplinary team to coordinate the full spectrum of patient care delivery in the home and dialysis setting.

Facilitates care delivery that meets standards of care set by the American Nephrology Nurses Association, attainment of patient outcomes that meet KDOQI goals and regulatory requirements of the Texas Department of Aging and Disability Services (DADS), other regulations and guidelines and the Centers for Medicare/Medicaid Services (CMS).

Maintain communication with personnel through staff meetings, conferences and written memorandums.

Organizes and prioritizes work to utilize resources wisely.

Completes assignments in a timely manner.

Addresses system problems and concerns with practical solutions.

 

Leadership and Teamwork:

Demonstrates leadership by consistent adherence to facility policies and procedures and all regulatory and industry standards.

Provides supervisor with informal feedback on an ongoing basis.

Participates with the facility leadership team to provide orientation and training for the safe and effective delivery of quality patient care that assures the standards outlined in the facility policy and procedure manual as well as regulations set forth by state and federal agencies.

Remains current in regulations and new developments impacting delivery of care.

Consistently demonstrates respect of individuals and cultural beliefs and lifestyles of patients/family members and coworkers.

Participates as an active member of the Quality Assessment Performance Improvement (QAPI) team through data collection, analysis, planning and implementation of quality improvement strategies to assure that the facility operates in a safe, efficient manner.

Practices risk management strategies and completes data collection forms as appropriate.

Oversees completion of clinic specific tasks as assigned.

Ensures the fulfillment of the Simply Dialysis, LLC. mission statement.

 

Safety:

Practices good body mechanics and correct methods of moving, lifting and transporting patients.

Adheres to all infection control policies.

Notifies supervisor and personnel of potential hazards.

Responsible and accountable for equipment maintenance, availability of adequate supplies and safety of the patient equipment and physical environment.
