JOB TYPE: Full-time/Permanent

SHIFT SCHEDULE: Days, Monday - Friday

SUMMARY: To oversee and provide direct medical care to patients with opioid use disorders, including performing medical evaluations, and providing periodic medical monitoring. This position will be responsible for developing medical protocol and procedures together with medical leadership.

REQUIRED CERTIFICATIONS AND LICENSES:

• Current unencumbered Nurse Practitioner license for the State of Oregon

• Unrestricted DEA certificate to prescribe controlled substances or Ability to obtain a DEA once the position is offered

• Basic Life Support (BLS) certification or ability to acquire within 90 days of hire

• Crisis prevention certification (such as CPI) within 90 days of hire

• A valid Oregon or Washington driver's license
