Position Summary:

Responsible for providing coordination services to youth at Bakari Center. Coordination services include involving residents and their families in planning, implementing and coordinating services and resources, and assisting with all aspects of transition planning.

The Residential Care Center is licensed under the Wisconsin Department of Children and Families Chapter 52 to provide a Type II residential treatment facility for 24 youth (ages 13-17).

Essential Functions:

  1. Provide coordination services that reflect the objectives of the treatment model and includes significant attention to youth engagement, positive reinforcement, and use of evidence-based best practices.
  2. Interview youth who have been referred to Bakari and complete the interview to be reviewed for admission decisions.
  3. Provide orientation programming for youth upon arrival to Bakari Center, inventory belongings and coordinate with families to obtain items such as clothing and personal care needs.
  4. Complete a case management assessment, and develop forms such as the Youth Care Plan, Aftercare plan and discharge summaries including domains of medical and health, educational and vocational, independent living skills, recreational interests, and permanency goals.
  5. Work collaboratively with the Clinicians to update the biopsychosocial domains of medical and health, educational and vocational, independent living skills, recreational interests, and permanency goals in the treatment plan.
  6. Provide resource referrals and coordination of services for youth and their families.
  7. Attend all court activities, appointments and/or arrange or provide transportation as needed.
  8. Coordinate and monitor education services for youth. This includes general education, vocational education, job readiness training, and tutorial services.
  9. Develop daily schedules for youth to help direct staff to activities.
  10. Co-facilitate extra-curricular school activities and participate in on-site programming for youth.
  11. Work with the youth’s team to develop weekend pass plans, including dates, times, supervision plan, and transportation, and communicate to all parties involved, weekly.
  12. Work in collaboration with the clinicians to ensure that annual medical and dental exams are completed for residents.
  13. Assist with medication administration, developing medication administration records monthly and as medications change, obtaining medication refills in collaboration with the Bakari Center Nurse, and preparing medications for weekend passes.
  14. Facilitate all aspects of discharge planning and work closely with MST-FIT team and Wraparound Milwaukee as required for transition planning.
  15. Maintain a daily log and progress notes for each youth that documents daily contacts.
  16. Review communication logs and narratives to generate weekly behavioral summaries to be shared with the youth’s team.
  17. Attend multi-disciplinary weekly staffing and monthly team meetings.
  18. Manage youth records/files ensuring they are compliant with licensing and funding requirements.
  19. Complete incident reporting requirements and notifications to funders and caregivers in collaboration with the management and clinical teams.
  20. Assist with managing information for quality assurance purposes including performance outcome measure data, center registry, and billing purposes.
  21. Keep appropriate documentation for all funding sources.
  22. Maintain communication liaison relationships with professionals related to Residential Care Center, Type II.
  23. Uphold and promote a positive and supportive culture for youth and staff.
  24. Maintain confidentiality within all written and verbal communications
  25. Adhere to state licensing requirements.
  26. Adhere to WCS Policies and Procedures.

Other Duties and Responsibilities: Provide coverage for absent staff and direct service to clients as needed. Other job-related duties as may be necessary to carry out the responsibilities of the position.

Job Qualifications:

Minimal Qualifications:

  • A bachelor's degree in social or behavioral science field (social work, sociology, psychology, speech communication or special education). Master’s degree preferred.
  • Two years field work experience or employment experience working with children and/or families.
  • Working knowledge of computer applications preferred (Windows and Microsoft Word).
  • Ability to display cultural competence by responding respectfully and effectively to people of all cultures, languages, classes, races, ethnic backgrounds, religions and other diversity factors in a manner that recognizes, affirms and values the worth of each client.
  • Knowledge and skills to work with population age 12-17.
  • Valid driver’s license, automobile, and insurance sufficient to meet agency requirements.
  • Meet all the employee requirements including references, criminal background check and driver’s license check.

Work Relationship and Scope: Reports directly to the Program Director. Has contact with a wide variety of individuals including youth and family members, other program staff, vendors, and other collateral contacts, neighbors, funders, Milwaukee County Department of Health and Human Services, Milwaukee County Division of Youth and Family Services, Children’s Court officials, Wraparound Milwaukee, the Department of Children and Families, Milwaukee Public Schools staff and administrators, staff of youth serving agencies, and the community.

Knowledge, Skills, and Abilities: Working knowledge of positive child/youth development; patience and understanding of difficult life situations; the ability to interact with escalating youth and/or parents in a calm and professional manner; ability to follow oral and written instructions; ability to respond appropriately in crisis situations; proficient with Microsoft Word and Excel; perform basic computer functions; uses time efficiently and effectively; ability to multi-task; sensitivity towards cultural, ethnic and disability issues.

Working Conditions:

  • State-Licensed Residential Care Center, Type II

• Local travel as required fulfilling essential functions of the position.

• Ability to take rotational 24-hour supervisory call responsibilities.

• Ability to manage young clients who have experienced a great deal of trauma and express trauma through behaviors.

Physical Demands: Duties require possible activity while monitoring or interacting with youth in the program. Staff may be required to use physical intervention in extreme situations. Driving required.
