The Registered Nurse Case Manager will be responsible for providing high-quality care for patients, coordinating care across multiple healthcare providers, and acting as a liaison between patients and their families. The RN Case Manager will work in a community setting, developing and implementing plans of care for patients, assist in the coordination of care across multiple providers, monitor patient progress, and evaluate the effectiveness of care plans.

• Assess the medical, psychological, and social needs of patients and develop a comprehensive plan of care that meets those needs
• Coordinate care services with other health care professionals to ensure continuity of care
• Perform clinical assessments of patients to evaluate their health status and determine appropriate interventions
• Develop and implement effective patient care plans
• Monitor patient progress and evaluate the effectiveness of care plans
• Provide patient education and counseling
• Provide support services to patients and their families
• Create and maintain accurate patient records
• Facilitate referrals to appropriate services and resources
• Manage patient discharge planning
• Participate in interdisciplinary care conferences and meetings

• Bachelor’s Degree in Nursing from an accredited college/university
• Current Registered Nurse (RN) license in the state of practice
• At least three years of nursing experience, with experience in case management preferred
• Excellent clinical assessment and communication skills
• Knowledge of medical terminology and disease processes
• Able to work independently and collaboratively as part of a healthcare team
• Able to prioritize multiple tasks and work under pressure
• Able to work with diverse populations
