We are seeking an experienced and compassionate Home Health Registered Nurse to provide high-quality nursing care to patients in the comfort of their own homes. The Home Health Registered Nurse will be responsible for assessing patient needs and developing individualized care plans to ensure that the highest standards of care are met. The ideal candidate will have excellent communication and problem-solving skills, and be patient-focused with a commitment to providing the best care possible.

• Assess patient needs and develop individualized care plans that include wound care, medication management, and patient and family education
• Monitor and evaluate patient progress and implement necessary changes to care plans as needed
• Administer medications, treatments, and therapies as prescribed
• Educate patients and families on health maintenance, disease prevention, and safety measures
• Collaborate with physicians, nurses, and other healthcare professionals to ensure optimal care
• Maintain accurate records of patient information and care plans
• Provide emotional support and guidance to patients and families

• Current Registered Nurse (RN) license
• Minimum two years of experience in home healthcare preferred
• Current CPR certification
• Knowledge of Medicare/Medicaid and other insurance requirements
• Excellent communication, problem-solving and organizational skills
• Able to effectively manage time and prioritize tasks
• Able to work independently and as part of a team
