BSW Social Woker

Dynamic and growing healthcare clinic in need of passionate case managers with a Bachelor's Degree in Social Work to provide care coordination for the vulnerable adult population in Greater Wayne County Michigan. We assist patients who suffer from mental illness, substance abuse, or who have a developmental disability to maximize their potential to live independently and thrive in the community of their choice. The right person for this job will be detail oriented and able to multi-task effectively, possess strong organizational skills, and be able to enter progress notes into the computer for all client encounters on a daily basis. Bachelor's degree and licensure (or license eligible) in Social Work (LLBSW, LBSW) Bi-lingual a plus! Spanish, Arabic, Sign-Language

Qualifications:

• Bachelor’s degree in Social Work or Human Services related field
• Has or is able to obtain required licensure
• Employee must have a valid Driver’s License with a driving record that is acceptable to the company’s insurance providers
• Employees must provide their own reliable transportation to and from the office workplace/work related locations and to transport members, as needed, (without delay) in a HIPAA compliant environment, with/without advanced notice
