Continuum is adding remote, nationwide positions for Social Work Care Navigators that will be responsible for resource referral, counseling, and coordination of care transitions for older adults with dementia and their caregivers.  

Position:  Care Navigator

Hours:  Flexible to your schedule

We want you to join our team as our Care Navigator because you:
