St. Mary's Medical Center is seeking a Part Time Social Worker (BSW).The Social Worker (BSW) exercises independent judgement and professional discretion in the practice of case management to a selected group of patients. The BSW collaborates with attending physicians and residents and serves as advocate to help patients and families navigate the acute episode of care. The BSW monitors patient's continuing stay for resource utilization and helps prepare them for a safe and timely discharge. The BSW is proactively uses collaborative problem solving skills to meet the needs of the patients and physician. The BSW provides a full range of medical social work services as needed and relies heavily on such resources as family, community, state and federal agencies as well as the patient's resources to assist the patient's return in a productive life.
