This can be 1 full day or 2 mornings/ week.  8 - 10 hours/ week.

Adult health care in Sunnyvale needs a part-time or contract LCSW or MFT for the following position:

POSITION DESCRIPTION:  As part of the Multi-Disciplinary team and in coordination with center’s MSW, conducts assessments and creates care plans for the elderly participants.

DUTIES:

  1. Participates in the initial and continuing assessments for the development of an individual plan of care by the Multidisciplinary Team.
  2. Maintains signed progress records in the participant's health record and documents all treatments as they are provided to participant.
  3. Provide three hours of consultation to center staff. Must work the minimum number of hours necessary to meet the needs of the participants.
