What You'll Do

As a valued member of our interdisciplinary hospice team, you'll:

  • Provide emotional support and counseling to patients and families navigating end-of-life care.
  • Complete psychosocial assessments and develop individualized plans to meet each patient's unique needs.
  • Connect patients and families with community resources, advance care planning, financial assistance, and supportive services.
  • Serve as an advocate, educator, and trusted guide throughout the hospice journey.
  • Collaborate with nurses, physicians, chaplains, hospice aides, and other team members to provide exceptional, patient-centered care.
What You'll Bring

Required Qualifications:

  • Bachelor's or Master's degree in Social Work (BSW or MSW).
  • Current Missouri Social Work license (or eligibility for licensure, as applicable).
