Sentara Health is seeking to hire a qualified individual to join our team as a Social Worker.
Overview:
The Social Worker BSW - Home Health performs a variety of casework and provides social work services to patients, families, and significant Must develop and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses.
Anticipated Status/Shift:
Part-Time (24 hours/week)
Day Shift (8AM-5PM)
Position Location:
Sentara Hospice Services - South Boston, VA (travel can include up to a 50-mile radius for position)
Salary Range: 25.60-42.67/hour

Education
  • Bachelor's Level Degree in Social Work (Required)

Certification/Licensure
  • Valid Driver's License (Required)
  • BLS (Required within 90 days of hire)

Experience
  • 1 year of social work experience (Required)
  • Previous Home Care/Hospice experience (Preferred)
