Position Summary

Provides direct and indirect patient care in a healthcare facility setting. Provides care that reflects initiative, flexibility and responsibility indicative of professional expectation with minimum supervision. Determines priorities of care based on physical and psychosocial needs, as well as factors influencing patient flow through the system. Communicates with outpatient clinic physicians about changes in patient's status, symptomatology and results of diagnostic studies. Is able to respond quickly and accurately to changes in condition or response to treatment.

Required Qualifications
  • Graduate of an accredited school of Nursing with up to six (6) months of Registered Nurse experience.
  • Current Registered Nurse (RN) license within the state of California and BLS certified.
  • Adheres to dress code, appearance is neat and clean.
  • Previous nursing experience in a similar healthcare facility setting preferred.
  • Commitment to HIPAA and ability to abide by standards of professional ethics and maintain confidentiality.
  • Actively participates in performance improvement and continuous quality improvement (QAQI) activities.
  • Complies with all organizational policies regarding ethical business practices. Communicates the mission, ethics and goals of the organization.
  • Adhere to an 8 hour shift requirement working 7 am and 6pm schedule.
  • Communicate clearly and concisely, both verbally and in writing and demonstrate a high level of listening skills.
  • Fluency in additional languages preferred.
  • Computer (PC) literate in general office functions such as MS word, access, email and preparation of spreadsheets.
  • Demonstrated commitment to the provision of services for the underserved and sensitivity working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, HIV (AIDS) infected, and/or physiologically impaired.
  • Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing.
  • Assist and support the Center/Department/Program to meet standards of High Reliability.
  • Must have valid driver's license and insurable driving record.


Essential Responsibilities
  • Provides direct patient care, evaluates outcomes, consults with other specialists as required and adjusts nursing care processes as indicated to ensure optimal patient care.
  • Performs a head-to-toe assessment on all patients and reassessments as per policy.
  • Adequately assesses and reassesses pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Attends annual review and department in-services, as scheduled; staff meetings as required, reads and returns all monthly staff meeting minutes.
  • Supports and maintains a culture of safety and quality.
  • Demonstrates knowledge of cardiac monitoring, identifies dysrhythmias.
  • Accurately assigns triage categories.
  • Answers telephone calls in a professional manner; refers patient problems to outpatient clinic physician; schedules patient appointments.
  • Maintains accurate and continued nursing documentation including patient histories, conditions, treatments, responses and assessment of changes.
  • Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and family. Functions as liaison between administration, patients, physicians and other healthcare providers.
  • Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care to the patients served. Able to interpret data about the patient's status in order to identify each patient's age specific needs and provide care needed by the patient group.
  • Interacts professionally with patient and family and involves patient and family in the formation of the plan of care.
  • Formulates a teaching plan based on identified learning needs and evaluates effectiveness of learning; includes family in teaching, as appropriate.
  • Maintains current knowledge of medications and their correct administration based on age of the patient and his/her clinical condition.
  • Follows the seven (7) medication rights and reduces the potential for medication errors.
  • Demonstrates knowledge of lab requirements, requisitions.
  • Maintains established outpatient clinic policies and procedures, objectives, performance improvement program, safety, environmental and infection prevention and control standards.
  • Maintains a safe, comfortable and therapeutic environment for patients and families in accordance with outpatient clinic standards.
  • Ensures an adequate stock of supplies and proper functioning of equipment.
  • Assists in cost containment through appropriate ordering and conserving of supplies and equipment.
  • Cooperates with other staff to achieve outpatient clinic objectives, maintain good employee relations.
  • Demonstrates knowledge of healthcare plans, procedural costs.
  • Demonstrates the ability to assist physicians with procedures in the outpatient clinic.
  • Leads one or more employees performing similar work; supervises temporary employees and employees who do not supervise; trains new employees; allocates and monitors work of others.
  • Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops; shows responsibility for own professional practice and ongoing education.
  • Performs other related duties as assigned or requested.

Physical Demands and Work Environment

The work environment is characteristic of a medical clinic environment. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit; use hands to handle, or feel. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment is usually quiet.

The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves or their homes. The employee may experience a number of unpleasant sensory demands associated with the client's use of alcohol and drugs, and the lack of personal care. The employee may also be exposed to bodily fluids (blood, urine) and hazardous chemicals.
