JOB DESCRIPTION:
The University of Vermont Health Network (UVMHN) was built with the foundational vision of High Value Care (HVC) and Clinical Integration. The SVP for High Value Care will partner with the EVP for Clinical Affairs to accelerate implementation of HVC goals. We will create and deliver value to our patients, providers, and communities. We will lead our region, and set a national example. HVC is the opportunity for our generation to meaningfully improve care outcomes in the face of rising costs, complexities, and inequities. It is our opportunity at UVMHN to demonstrate how care delivery organizations can uniquely lead in HVC. We will measure overall progress through the value equation of outcomes relative to costs, and our daily progress at the point-of-care.
At UVMHN we are focusing on three functional areas for HVC: (i) Risk-based contracting, (ii) Population health services, (iii) and Care delivery. Our Population Health Services Organization (PHSO) comprises informatics, analytics, documentation and coding, and care management. Care delivery prioritizes quality improvement, health equity, patient access, and clinical workflows. Some of our recent achievements include:
• Shifting away from fee-for-service reimbursement to value-based and prepaid revenue models
• Developing and participating in alternative payment models in Vermont and New York through waivers and payer contracting, accepting risk and accountability with attributed populations
• Building accountable care organizations serving Vermont and Northern New York
• Developing partnerships with value-oriented payers
• Developing partnerships with Federally Qualified Health Centers (FQHCs)
• Transitioning our Jeffords Institute to more directly support population health efforts
• Launching a Data Management Office (DMO)
• Investing in and implementing enabling technologies such as EPIC, Axiom, and Premier Connect
The inaugural SVP for High Value Care leader advanced many of the initiatives above, and convened a team of experts and innovators within UVMHN. The EVP for Clinical Affairs position has been designed for accountability in HVC and to partner and support the SVP for HVC. Major opportunities include operationalizing connections among the three HVC functional areas; making data-driven and timely decisions; engaging and motivating physicians, advanced practice providers, and nurses at the point-of-care; and deploying HVC tactics not only for performance on risk-based contracts, but also for performance on legacy fee-for-service contracts and to strengthen Network clinical integration.
