The Family Caregiver Support Intake Specialist is responsible for utilizing a variety of assessments and screening tools to develop initial care plans for newly enrolled individuals in the Family Caregiver Support Programs. The Family Caregiver Support Intake Specialist provides information and referrals to community resources, long-term services, and supports that best fit client needs. They also provide client advocacy and program education to newly referred individuals. The Caregiver Support Intake Specialist engages in community outreach.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Work with newly enrolled referrals by conducting Presumptive Eligibility screenings, initial screenings and initial functional assessments.
  • Complete timely and accurate initial plans of care while ensuring the appropriate services are being offered to clients.
  • Provide referrals to contracted vendors to link client to services.
  • Promote the program by participating in outreach activities.
  • Advocate for clients when appropriate.
  • Adhere to program standards and guidelines.
  • Complete all documentation and entries into State databases within the required timeframe set forth by State standards.
  • Establish and maintain a good working relationship with FBH staff and community partners. Perform other related duties as required.
  • Comply with all State WAC and RCW requirements and FBH policies and procedures.
  • Maintain professionalism when interacting with clients, staff, and agency partners.
  • Other duties as assigned


QUALIFIED CANDIDATES WILL HAVE:

REQUIRED:

Staff administering the full TCARE® assessment/consultation and service planning must meet the minimum qualifications of an AAA Case Manager and certification in the State of Washington. These qualifications include the following minimum education and experience requirements:
  • A Master's degree in behavioral or health sciences and one year of paid on-the-job social service experiences; or
  • A Bachelor's degree in behavioral or health sciences and two years of paid on-the-job social service experience; or
  • A Bachelor's degree and four years of paid on-the-job social service experience.
  • Must apply for Agency Affiliation within 30 days of hire.


KNOWLEDGE, SKILLS, & ABILITIES:
  • Ability to communicate and engage effectively with individuals of varied backgrounds, cultures, education level and socio-economic status.
  • Knowledge of elder care and disabled adult caregiving issues.
  • Ability to perform face-to-face assessments and screenings.
  • Knowledge of data entry and computer systems to complete tasks
  • Proficiency with Microsoft Office suite and experience with database systems.
  • Ability to complete administrative tasks and organize and prioritize tasks.
  • Must have reliable transportation, insurance and an active drivers' license and be willing to use personal vehicle in the course of work.
  • Experience with leading large groups and public speaking.


PHYSICAL, SENSORY & ENVIRONMENTAL:

Work is generally performed in an office setting which may involve sitting and use of computers for four to eight hours per day. May be required to lift 10 - 20 pounds. Work may be performed out of office and travel may be needed to outside areas.

Blood-borne Pathogen Category III - No predictable exposure to blood-borne pathogens

PAY RANGE (Hourly/ Non-Exempt): $25.89- $31.53

*All pay is based on longevity at Frontier Behavioral Health; thus, the starting pay for this position is $25.89 per hour.
