Job Title: Medical Assistant-Health Coach
Classification: Full Time/ Non-Exempt
Summary: Work alongside medical providers to perform clinical and administrative duties to assure patients receive optimal care and reduce barriers to positive medical outcomes.
Responsibilities:
Rooms patients and triages for medical appointments, to include taking vitals, medical history, assessing focus of medical visit.
Completes all patient referrals, according to provider requests.
Manages patient’s medication refill requests.
Assist in management of preventive care, such as vaccinations and diagnostic tests.
Contact patients to schedule planned care visit, via letter or direct telephone contact as appropriate. Provide patients with education.
Responsible for establishing trusting relationships with patients while identifying, connecting and supporting social determinants of health needs.
Provide care coordination, evaluation, and advocacy for patients, in regards to social services, such as transportation, food pantry, and other supports programs, which are offered in-house or through external agencies.
Provide ongoing navigation with patients to assure patient satisfaction, evidenced by patient retention.
Maintain current information on available area resources that can benefit patients.
Assist patients in applying for patient’s assistance programs through pharmaceutical companies.
Assist individuals with identified social services through internal or external resources.
Track outcomes of social services referrals, identifying barriers and assisting patients in overcoming such barriers.
Track and manage in-house voucher system for financial support of medications and labs.
Following-up with patients via phone calls, home visits and other necessary means.
Exhibit excellent working relations with community members, patients, visitors and team members.
Work closely with providers and direct care staff to assure patient’s needs are being met.
Other duties as required by supervisor.
Additional Expectations:
To be proficient in using the EMR to document all patient data.
Maintain a helpful positive attitude that lends itself to team work in meeting patient goals.
Qualifications/Experience:
Candidate must have a BA in social or health sciences; or be a certified Medical Assistant.
Preferred experience in working in a FQHC or similar health center.
Time/Physical Requirements
Hours based on clinic schedule, to be determined.
Some evening and weekend activities may require flexing schedule
Tasks occur in both medical office and clinic setting
Able to lift 25lbs and perform normal duties associated in a clinic and office setting.
CBWF was founded in 1986, as the Coastal Bend AIDS Foundation, by a group of concerned citizens, who wanted to help people dying of AIDS. The group started by delivering food and blankets from the trunk of a car. As the disease became more chronic rather than acute, and funding became available for additional services, the agency grew to provide additional support services for those living with HIV and AIDS.
In 2012, the board of directors decided to change the name of the agency, to better fit the array of services provided. Today, the Coastal Bend Wellness Foundation still honors its founding members by providing HIV/AIDS specific medical treatment and support services, but also encompasses programs for the entire community.
In 2015, CBWF became a federally qualified health center. We now are proud to be able to provide primary health care to anyone, regardless of the ability to pay or insurance status. Since its inception, CBWF has consistently pursued development of service programs that translated its mission statement from a planning concept into action, moving it towards the reality of a seamless continuation of health care and support services for those most in need.
