Assistant Director of Clinical Services

The Assistant Director of Clinical Services is responsible for ensuring that patient care is coordinated and managed appropriately, and that care and services are delivered appropriately as well as the supervision of clinical personnel.

Essential Job Functions/Responsibilities:

1. Assists the Clinical Director with referral review as needed, including disciplines required, to determine home care needs. Assists assigning appropriate clinicians to case if needed.
2. Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians as appropriate. And participates in the Administrative on call rotation after hours if needed or as directed.
3. Reviews patient’s clinical diagnosis, medications, procedures, and clinical course.
4. Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in creating and developing the plan of care.
5. Plans and schedules case conference meetings with clinicians and organization personnel to facilitate coordination of care. Creates case conference agenda and case review list based upon prior week’s hospitalizations, admissions, recertifications and resumptions of care. And other items that may be added as indicated.
6. Collaborates and completes annual performance reviews for clinical staff.
7. Directly supervises agency preceptors for clinical staff, closely monitoring the effectiveness of the preceptor program and making recommendations to the Clinical Director for any changes needed to the preceptor program if/when necessary. And to function as a back up preceptor and/or to assist with new hire orientation and training if needed or as directed.
8. Assists with visit coverage if a clinician calls off and a visit (s), start of care, resumption of care or recertification of care needs to be completed. Not to carry an individual case load, but to assist when/if necessary or as directed.
9. Performs other duties and activities as delegated by the Clinical Director

Position Qualifications:

-Registered nurse with current licensure to practice professional nursing in the state.

-RN BSN, Graduate of an accredited nursing program.

-Two years management or supervisory experience in a health care setting, preferably home care is preferred.

-Complies with accepted professional standards and practice.

-Complies and maintains current CPR certification.

-Demonstrates excellent observation, verbal and written communication skills.
