Clinical Case Manager Job Summary
Our mental health clinic is seeking a Clinical Case Manager to oversee the care of our patients. In this role, you will work with individual patients in a crisis intervention capacity and help them through mental health crises, so you need several years of experience as a counselor or social worker. You will also provide interventional treatment planning services for these patients to help them overcome their issues in the long term. Our ideal applicant has at least a bachelor's degree in psychology or social work with some experience with behavior analysis. Experience as a youth worker is also beneficial.
Clinical Case Manager Duties and Responsibilities
- Meet with patients to assess their mental health status
- Develop a treatment plan for long-term progress
- Provide immediate crisis intervention care
- Provide regular audits to assess progress
- Keep track of patient records
Clinical Case Manager Requirements and Qualifications
- Bachelor's degree in psychology or social work; master's degree preferred
- Social work or youth work experience
- Behavior analysis and problem-solving abilities
