Job description

Job Title: Patient Care Coordinator (Telehealth / Virtual Care)

Location: New York NY 10017 [ONSITE ROLE]
Duration: 13 weeks
Schedule: (11:00 PM - 7:00 AM, every other weekend)


Role Summary

The Patient Care Coordinator supports hospital-based telehealth and remote patient monitoring operations. This role focuses on continuous virtual patient observation, clinical escalation, and coordination of telehealth workflows to ensure patient safety and high-quality virtual care delivery.


Key Responsibilities

  • Monitor patients remotely for clinical and behavioral changes and escalate concerns to nursing/physician teams
  • Support telehealth consultations, scheduling, and Epic-based virtual visit workflows
  • Ensure smooth execution of virtual care sessions and troubleshoot platform/equipment issues
  • Document patient behavior, interventions, and safety events (paper documentation)
  • Perform quality checks, track performance metrics, and support program improvement initiatives
  • Collaborate with clinicians, leadership, and technical teams on telehealth operations and workflows
  • Provide training and real-time support for telehealth tools and processes
  • Maintain compliance with HIPAA and hospital safety protocols

Requirements

  • Minimum 3 years of direct patient care experience
  • At least 2 years of telehealth or remote monitoring experience
  • Experience with EMR systems (Epic preferred)
  • Background in patient safety, quality improvement, and clinical escalation
  • Strong communication, multitasking, and problem-solving skills
  • Associate's or Bachelor's degree in a health-related field (preferred)
