As the Transitional Care Manager, you'll contribute to our success in the following ways:

  • Executes strategic plans and strives to obtain departmental goals.
  • Develops and implements day to day program procedures and practices. Communicates changes to these procedures and practices in a timely manner as the program grows and evolves.
  • Acts as a consultant between service line leaders and team to ensure effective collaboration.
  • Develops policies and procedures to enhance the Transitional Care program.
  • Provides leadership for department and staff.
  • Serves as an educator to internal and external key stakeholders, a resource person, and assists in orientation of all new hire employees.
  • Actively identify and address conflicts involving team, using effective communication and negotiation skills to facilitate resolution.
  • Maintains a thorough understanding of applicable insurance rules and regulations, on program financial situations, goals, budget, and outcomes. Continuously looks for cost savings opportunities.
  • Produces dashboards and data to executive staff for tracking outcomes of the program.
  • Responds to patient and/or family complaints and works to resolve them in accordance to policy.
  • Treats all patients and caregivers with respect and courtesy.

What is Required?

  • Bachelor's degree in School of Nursing, preferred
  • 3+ years nursing experience in hospice, home health, geriatrics, or general med-surg nursing
  • Certified Hospice and Palliative Nurse (CHPN) or willingness to obtain the certification within 2 years.
  • Must have a dependable vehicle, valid drivers license and current auto insurance in accordance with the laws of the state.

You will report to the Senior Manager, Transitional Care.
