Shift: Days | 8-hour shifts
Pay Range: $30.11+/hour (Based on education and experience)
Relocation Assistance: Up to $5,000 available


Job Overview

We are seeking an experienced Care Manager to join an acute care hospital team. This role is ideal for a nursing professional with strong case management experience who is passionate about coordinating patient care, improving outcomes, and ensuring safe, efficient transitions throughout the healthcare journey.


The Care Manager will assess patient needs, coordinate care plans, collaborate with healthcare providers, and support patients and families by connecting them with appropriate resources and services.
