Job description

Support Coach

The Support Coach is a direct care level person responsible for delivery of a full range of support services to individuals impacted by a variety of behavioral health issues, recognizing that the individuals we serve often have experienced trauma that affects their development and adjustment. The Support Coach is primarily responsible for facilitating the maximum level of independent functioning of the clients served by teaching and role modeling skills to help facilitate independence. The Support Coach will demonstrate a warm and welcoming, empathic, hopeful attitude, conveying a philosophy of dual recovery. The Support Coach will be trained in trauma-informed care and will provide services in an environment that is sensitive and responsive and that will promote improved wellbeing because of services received. The Support Coach reports to the Adult Residential Administrator.

Education:

  • High school diploma or equivalent required.

Preferred Qualifications:

  • A minimum of six (6) months full-time employment experience as an agency homemaker, certified nurse aide, or a minimum of one (1) year experience caring for children, elderly, or physically/mentally ill individuals.
  • Successful completion of formal training in nursing arts or as a Certified Nurse Aide, or Home Health Aide can substitute for the qualifying experience

Responsibilities:

  • During the shift, make sure every client is where they should be, encourage them to join in scheduled activities and facilities, and accurately record client actions and observations in daily progress notes.
  • Prepare and serve appealing meals and snacks, per established menu plans and within timelines.
  • Accurately prepare, distribute, and document medications per each client's medication administration record (MAR) and per physician's orders.
  • Help, instruct, and guide clients in carrying out daily tasks in accordance with their Intensive Residential Treatment Services (IRTS) program and treatment plan.
  • Teach and serve as a model for clients on how to control their conduct and use practical coping mechanisms in the context of building relationships.
  • Plan and engage clients in social skills activities with Support Coach and peers to encourage purposeful activity and reduce isolation.
  • Consistently complete routine cleaning, housekeeping and other assigned duties per Adult Residential Administrator or designee.
  • Accurately finish the paperwork assigned before starting the following shift, and share any client problems or issues with your supervisor when your shifts change. Every necessary document is finished within the allotted time frames and in compliance with the program's requirements, CARF, state and federal regulations, and quality standards.
  • Immediately contact the Adult Residential Administrator (or designee), in the event of a) any client or staff injury or need for ER/hospitalization b) death of a client, b) suspected use or neglect of client, c) any other situation requiring notification of the Director of Community Care as dictated by policy and procedure.
  • Performs all other duties as assigned.

Physical Requirements:

  • Normal direct care environment.
  • Requires evening and/or weekend work.
  • Some travel may be required for appointments and community outings.
  • Typical physical demands include frequent standing, bending, stooping, and occasional lifting, up to 50 pounds.
  • Requires normal range of hearing and manual dexterity sufficient to assist clients in day-to-day living routine.
  • Requires ability to work under stressful conditions and to work irregular hours.
  • Requires use of a computer.
  • TB test upon hire.
