What is an SSVF case?

SSVF Case Managers are professionals who work within the Supportive Services for Veteran Families (SSVF) program to help veterans and their families who are homeless or at risk of becoming homeless. Their main role is to assess clients' needs, connect them with housing resources, and provide ongoing support services such as counseling, financial planning, and assistance with benefits. SSVF Case Managers collaborate with local agencies, landlords, and community organizations to secure stable housing and promote long-term self-sufficiency for veterans and their families.

What skills and qualifications are needed to thrive as an SSVF case?

To thrive as an SSVF Case Manager, you need a background in social work, counseling, or a related field, often supported by a relevant degree and experience with vulnerable populations. Familiarity with case management software, the Homeless Management Information System (HMIS), and knowledge of VA and housing assistance programs are important technical requirements. Strong interpersonal communication, problem-solving abilities, and cultural competence help build trust with clients and coordinate resources effectively. These skills are crucial for successfully guiding veterans toward stable housing and support services, ensuring positive outcomes in challenging circumstances.

What are common challenges faced by SSVF cases when working with veterans, and how are these addressed?

SSVF Case Managers often encounter challenges such as helping veterans overcome housing instability, addressing co-occurring mental health or substance use issues, and navigating access to community resources. Building trust with clients who may have had negative experiences with institutions is another frequent hurdle. To address these challenges, case managers use trauma-informed approaches, collaborate closely with multidisciplinary teams, and leverage community partnerships to provide holistic and coordinated support. Ongoing training and peer support are also essential in managing caseload stress and ensuring effective service delivery.

Position Summary

The SSVF Case Manager provides comprehensive case management and supportive services to Veterans and Veteran families who are homeless or at imminent risk of homelessness through the U.S. Department of Veterans Affairs Supportive Services for Veteran Families (SSVF) Program. Working collaboratively with Veterans, landlords, the Department of Veterans Affairs, and community partners, the Case Manager provides advocacy, crisis intervention, and ongoing support to promote long-term housing stability, self-sufficiency, and improved quality of life.

This position is primarily office-based with frequent travel throughout the community to conduct participant visits, outreach, and meetings with landlords and partner agencies. The work requires regular interaction with Veterans, families, community providers, and government agencies in a fast-paced environment that demands professionalism, flexibility, sound judgment, and effective time management.

  • Develop, implement, and monitor individualized Housing Stability Plans and service goals.
  • Provide ongoing case management through office visits, home visits, telephone contacts, and community outreach.
  • Assist participants in obtaining and maintaining safe, affordable permanent housing through housing searches, landlord engagement, lease coordination, and housing stabilization services.
  • Coordinate temporary financial assistance in accordance with SSVF guidelines.
  • Connect participants to appropriate community resources and supportive services, including VA healthcare and benefits, behavioral health services, employment and vocational programs, financial literacy and budgeting, income assistance, childcare, transportation, and other community resources.

  • Conduct outreach to unsheltered Veterans and maintain collaborative relationships with landlords, property managers, the VA, healthcare providers, and community agencies.
  • Maintain accurate, timely, and complete participant documentation in HMIS and other required databases while ensuring compliance with SSVF, HIPAA, federal, state, and agency requirements.
  • Prepare reports, reimbursement documentation, and other required program records.
  • Participate in case staffing meetings, trainings, and professional development activities.
  • Represent Fairbanks Rescue Mission professionally while maintaining participant confidentiality and appropriate professional boundaries.
  • Perform other duties as assigned.
