Job Description
Shriners Northern California is looking for a California Licensed Social Worker to join our team! The Social Worker position requires a thorough understanding of the clinical, financial, administrative, and psychosocial components of care management. The Social Worker is responsible for providing psychosocial assessments, planning, interventions and follow-up for patients and family members at Shriners Children's across the continuum of care. These services are provided face to face or telephonically; to facilitate the creation and adherence to a treatment plan and optimize Patient health in conjunction with the interdisciplinary team; this position pro-actively addresses potential or actual needs identified through screening to ensure developmental, level of care and health status transitions are safe, seamless, and coordinated.
Key Responsibilities:
- Bio-psychosocial assessment
- Crisis intervention
- Brief Suicide Screening Assessment
- Participation in Multidisciplinary Rounds/Conferences
- Patient and family support and education
- Referrals to community based services
- Discharge and transition planning
Required Qualifications:
- CA LCSW
- Masters Degree in Social Work
- CMC, ACM or ANCC (within 12 months of hire or eligibility)
Preferred Qualifications:
- Bilingual
- 3+ years pediatric healthcare experience
The pay range for this position is $38.58 - $74.75. Compensation is determined based on years of relevant experience and departmental equity.
