This position allows for a remote work setting while being connected to our hospice center, where interdisciplinary teams focus on holistic patient care. • Provide compassionate psychotherapy to patients with diverse mental health conditions in a hospice setting, including anxiety, depression, grief, and adjustment to terminal illness.
• Conduct comprehensive mental health assessments to evaluate patients' emotional, behavioral, and social functioning effectively.
• Develop, implement, and evaluate personalized treatment plans grounded in evidence-based practices, ensuring cultural competence and sensitivity to the unique needs of each patient.
• Regularly collaborate with an interdisciplinary team of healthcare professionals, including physicians and psychiatrists, to ensure holistic and coherent patient care.
• Monitor and document patient progress through ongoing evaluations, adapting treatment strategies based on observed responses and feedback from other healthcare providers.
• Educate patients and their families about mental health challenges, available treatment options, and community resources, fostering a supportive environment for informed decision-making.
• Uphold ethical standards and professional guidelines, ensuring patient confidentiality and advocating for their rights and needs within the healthcare system.

This role particularly focuses on patients receiving hospice care, often facing complex psychological challenges as they navigate the emotional ramifications of terminal health conditions. The LCSW will work closely with patients experiencing grief, anxiety about impending loss, and fear of the unknown, while also providing support for family members who are impacted by these emotional battles. • A Master's degree in Social Work from an accredited institution is mandatory.
• Current state licensure as a Licensed Clinical Social Worker (LCSW) is required.

• Exceptional interpersonal and communication skills, aiding in effective relationship building with patients, families, and healthcare professionals.
