This position allows for a remote work setting while being connected to our hospice center, where interdisciplinary teams focus on holistic patient care. You will engage with our practitioners, healthcare staff, and patient family members virtually, maintaining professional communication guided by our organizational values. Our operational hours are Monday to Friday from 9:00 AM to 5:00 PM, creating a structured yet flexible environment that promotes work-life balance.

• Provide compassionate psychotherapy to patients with diverse mental health conditions in a hospice setting, including anxiety, depression, grief, and adjustment to terminal illness.
• Conduct comprehensive mental health assessments to evaluate patients' emotional, behavioral, and social functioning effectively.
• Develop, implement, and evaluate personalized treatment plans grounded in evidence-based practices, ensuring cultural competence and sensitivity to the unique needs of each patient.
• Regularly collaborate with an interdisciplinary team of healthcare professionals, including physicians and psychiatrists, to ensure holistic and coherent patient care.
• Monitor and document patient progress through ongoing evaluations, adapting treatment strategies based on observed responses and feedback from other healthcare providers.
• Educate patients and their families about mental health challenges, available treatment options, and community resources, fostering a supportive environment for informed decision-making.
• Uphold ethical standards and professional guidelines, ensuring patient confidentiality and advocating for their rights and needs within the healthcare system.

This role particularly focuses on patients receiving hospice care, often facing complex psychological challenges as they navigate the emotional ramifications of terminal health conditions. The LCSW will work closely with patients experiencing grief, anxiety about impending loss, and fear of the unknown, while also providing support for family members who are impacted by these emotional battles. By promoting mental health wellness, you will play a crucial role in enhancing the overall quality of life for patients in their final days.

• A Master's degree in Social Work from an accredited institution is mandatory.
• Current state licensure as a Licensed Clinical Social Worker (LCSW) is required.
• At least 2 years of experience in providing psychotherapy and mental health services, specifically for patients dealing with chronic illness or end-of-life issues.
• Exceptional interpersonal and communication skills, aiding in effective relationship building with patients, families, and healthcare professionals.
• Proficient clinical assessment skills, with the ability to formulate evidence-based treatment plans tailored to individual patient needs.
• Familiarity with electronic health records (EHR) systems and healthcare software for managing and coordinating care seamlessly.

• Knowledge of community resources for mental health support and an ability to navigate these effectively for patient referrals.
