WeCare is actively seeking a motivated RNAC for our community in PA. The RNAC is responsible for coordinating the Resident Assessment Instrument (RAI) process, including the Minimum Data Set (MDS), care planning, and quality assurance activities. The ideal candidate will have strong clinical assessment skills, knowledge of Medicare and Medicaid regulations, and a commitment to providing high-quality care to our residents.

Key Responsibilities
  • Coordinate the RAI process, including completion of the MDS and care planning
  • Ensure accuracy and completeness of resident assessments
  • Collaborate with interdisciplinary team members to develop individualized care plans
  • Monitor and evaluate the effectiveness of care plans
  • Ensure compliance with Medicare and Medicaid regulations
  • Provide education and training to staff on the RAI process and care planning
  • All other job duties given by administration 


Skills, Knowledge and Expertise
  • Current RN license in the state of PA
  • Certification in RAC-CT preferred
  • Strong clinical assessment skills
