Registered Nurse Assessment Coordinator

The Allure Group is looking for an RN Assessment Coordinator to join the team at our Skilled Nursing Facility in Brooklyn, NY!

The Allure Group is a consortium of six rehabilitation centers across Brooklyn and Manhattan, NY, specializing in post-acute and sub-acute skilled nursing and rehabilitation. As an RN Assessment Coordinator at The Allure Group, you would be part of a winning team doing meaningful work to improve the quality of life, health, and comfort of residents.

RN Assessment Coordinator Responsibilities
  • Conduct and coordinate the completion of MDS assessments
  • Maintain and update all care plans and assessments
  • Examine and review residents' medical history
  • Inform other nursing staff personnel of any persistent issues
  • Participate in the admission process while assessing prospective residents' nursing needs and appropriate placement
  • Coordinate case management process
  • Ensure that the facility adheres to all regulatory requirements and standards of practice in relation to the MDS care planning process
  • Cooperate with inter-departmental personnel to ensure services can be adequately maintained to meet the needs of the residents
RN Assessment Coordinator Requirements
  • Valid, current RN license
  • Previous experience in long-term care or skilled nursing facility
  • One-year job-related experience in the MDS process
  • Familiarity with various computer software
