Key Responsibilities
  • Conduct thorough assessments of seniorsโ€™ psychosocial needs, taking into account emotional, mental, and social factors that may affect their health and well-being.
  • Collaborate with clients and their families to develop individualized care plans tailored to their specific goals, preferences, and cultural backgrounds.
  • Provide counseling and emotional support to seniors as they navigate life transitions, such as relocation to assisted living, loss of a spouse, or health challenges.
  • Advocate for client rights, helping them connect to community resources, benefits, and services that enhance their quality of life.
  • Conduct regular virtual visits to monitor clientsโ€™ well-being, assess their living conditions, and address any potential safety concerns or required interventions.
  • Facilitate effective communication among family members involved in the clientโ€™s care, mediating conflicts to ensure a cohesive approach to support.
  • Educate clients and their families about community resources, social support programs, and benefits that can foster greater social engagement and wellbeing.

Qualifications
  • Master's degree in Social Work from an accredited institution.
