Key Responsibilities
  • Conduct in-depth assessments of clients' emotional, mental, and social needs to develop comprehensive care plans tailored to their preferences.
  • Provide expert counseling and emotional support, guiding seniors and their families through transitions such as relocation to assisted living or coping with loss.
  • Collaborate with a multidisciplinary team comprised of healthcare professionals, caregivers, and family members to ensure a holistic approach to care.
  • Monitor clients’ conditions through regular check-ins, adapting care plans as necessary to meet changing needs.
  • Advocate for client rights and services, facilitating access to local resources that can enhance their quality of life.
  • Document all client interactions and progress in a secure electronic health record (EHR) system, ensuring compliance with state regulations and ethical standards.
  • Educate clients and families about available community resources, connecting them with support groups, wellness programs, and recreational activities.
  • Participate in ongoing training and professional development opportunities to enhance your skills and knowledge in the field of social work.
Patient Population / Client Focus

In this role, you will primarily serve seniors in various stages of life, many of whom may be facing significant health challenges, cognitive decline, or social isolation. Your work will involve not only individual assessments but also the facilitation of family communications to ensure a comprehensive understanding of the client's situation. By engaging with older adults, you will play a crucial role in improving their quality of life and ensuring they maintain as much independence as possible.

Qualifications
  • A Master’s degree in Social Work from an accredited institution.
  • Current and valid licensure as a Master Social Worker (LMSW) in Illinois.
  • A minimum of two years’ experience in social work, with a preference for candidates with experience in geriatric care or community services.
  • Proficient understanding of social work principles, community resources, and issues impacting the aging population.
  • Exceptional assessment skills, complemented by strong counseling and problem-solving abilities.
  • Demonstrated empathy, compassion, and the ability to build rapport with elderly clients and their families.

Compensation and Benefits

The compensation for this position ranges from $60,000 to $100,000 annually, commensurate with experience and qualifications.
