Sun Valley Consultation Services is seeking a dedicated Full-Time Licensed Clinical Social Worker (LCSW) to join our remote team. This position is fully remote, allowing for flexible scheduling and the ability to establish your own working hours while ensuring timely and effective patient care. The role is uniquely situated within our hospice center, providing a compassionate approach to patients experiencing emotional difficulties associated with terminal illness and end-of-life care. Collaborating with a dedicated team of healthcare professionals, the LCSW will navigate the emotional and psychosocial aspects of their patients' experiences.

The LCSW will report directly to the Social Work Manager and be responsible for a range of duties designed to improve the psychosocial functioning of patients. Key tasks include:

• Conducting comprehensive bio-psychosocial evaluations to identify barriers to treatment and enhance patient care.
• Collaborating with medical and support staff to create integrated care plans aimed at promoting compliance and improving health outcomes.
• Developing and implementing individualized treatment plans utilizing evidence-based therapeutic modalities both in one-on-one and group settings.
• Documenting patient interactions diligently in medical records according to departmental standards, ensuring a high level of care coordination.
• Providing education and resources to patients and families, helping them navigate medical recommendations and integrate social work interventions effectively.
• Assisting patients and families in accessing community resources tailored to their advocacy needs, thereby enhancing social support structures.
• Maintaining continuous professional development through training and attending relevant workshops to stay abreast of best practices in clinical social work.

This position particularly focuses on serving patients in a hospice setting, addressing the complex psychosocial challenges that arise during end-of-life care. The ideal candidate will demonstrate a deep understanding of the emotional, cultural, and developmental factors affecting patients and their families during transitional times. Those served may have varying backgrounds and needs, thus requiring a compassionate, tailored approach to each individual.

To qualify for this role, candidates must possess:

• A Master's Degree in Social Work from an accredited graduate program.
• A current and valid License as a Clinical Social Worker (LCSW) from the State Board of Social Work Licensure.
• A strong understanding of the NASW Code of Ethics and relevant standards of practice.
• Exceptional interpersonal skills coupled with the ability to navigate sensitive discussions related to mental health and end-of-life issues.

While the minimum requirement is six months to one year of clinical experience, preferred candidates will have prior experience in health or mental health settings, particularly those focused on hospice care. Familiarity with working with diverse populations and understanding age-related issues will be advantageous in effectively supporting patients.

This position offers a competitive salary ranging from USD 85,000 to 100,000 annually, depending on experience.
