Key Responsibilities

  • Conduct comprehensive patient intakes, focusing on diagnostic and functional evaluations to create tailored treatment plans.
  • Deliver brief, focused interventions for patients seeking mental health support, guiding them towards positive outcomes.
  • Provide recommendations to the primary care team regarding patients' treatment goals and ensure appropriate referrals to specialty mental health facilities when necessary.
  • Monitor patient progress and follow up to make necessary adjustments to treatment plans.
  • Develop relapse prevention strategies and support patients in achieving stable functioning in their daily lives.
  • Document detailed patient progress and treatment plans within electronic health records to maintain continuity of care.

  • Empower patients by educating them on self-management skills, coping strategies, and healthy lifestyle choices.
