Key Responsibilities
  • Conduct thorough assessments and develop individualized treatment plans tailored to the unique needs of each client.
  • Provide direct counseling services through virtual platforms, utilizing evidence-based practices to ensure the highest quality of care.
  • Collaborate with team members to create and implement effective crisis plans, positive behavior support strategies, and medication management guidelines.
  • Complete administrative documentation promptly, ensuring compliance with regulatory standards and maintaining client confidentiality.
  • Actively engage in licensing and accreditation processes, demonstrating adherence to established professional standards.
  • Develop and maintain strong communication with state agencies and community partners to enhance service delivery and expand resources available to clients.
  • Participate in ongoing professional development activities, including attending relevant conferences and trainings.
  • Promote and embody the mission and values of Grace Hospital both within the team and in the community at large.
  • Perform additional duties as directed by the Clinic Director to meet the evolving needs of our clients and organization.

Qualifications
  • A Master's Degree in a behavioral health field is required, ensuring a solid foundation of clinical knowledge.
  • Candidates must hold an independent LMHC license recognized by the state of Connecticut.
