Big Leap Family Guidance Centers is seeking a dedicated and compassionate Licensed Professional Counselor (LPC) to join our team in a remote capacity. You will collaborate with a multidisciplinary team focused on integrated healthcare, ensuring that mental health services are effectively coordinated with primary medical care. • Conduct comprehensive patient intakes, focusing on diagnostic and functional evaluations, to recommend personalized treatment goals.
• Provide brief, focused therapeutic interventions for patients requiring mental health support, adhering to established community health clinic guidelines.
• Collaborate with primary care providers to identify patients needing additional specialty mental health services and coordinate appropriate referrals.
• Monitor patient progress, initiating follow-ups to determine treatment effectiveness and adjust strategies as needed.
• Develop relapse prevention plans for patients and assist in maintaining their stability through ongoing support and resource linkage.
• Document patient progress accurately in treatment charts, ensuring compliance with professional standards and clinic protocols.
• Offer consultation to clinic management and team members regarding behavioral health services and program evaluation.
• Assist patients in adhering to any prescribed medical treatments, equipping them with coping strategies for managing medication side effects.
• Foster a therapeutic environment where patients feel safe and empowered to participate actively in their recovery journey.
• Contribute to a collaborative team atmosphere by maintaining effective relationships with colleagues, participating in interdisciplinary meetings, and sharing best practices.

You will work with a diverse patient population that includes individuals of various ages and backgrounds who are navigating a range of mental health issues. Your efforts will focus on helping patients with a spectrum of needs—from anxiety and depression to more complex psychosocial challenges—as you provide support that aligns with their personal treatment goals.

• A Master's Degree in Clinical Counseling is required.
• A current Licensed Professional Counselor (LPC) license in good standing.
• Extensive knowledge of mental health assessment, treatment planning, and psychotherapeutic techniques.
• Ability to maintain thorough clinical records and prepare reports, demonstrating comprehensive understanding of treatment processes.
• Exceptional interpersonal skills to establish and maintain effective relationships with patients, families, and team members.
• Strong analytical skills with the ability to assess individual patient needs and make informed treatment decisions.
• Excellent communication skills and the ability to work collaboratively in an interdisciplinary team environment.

• Bi-lingual English-Spanish proficiency is preferred but not required, aiding in communication with our diverse client base.
• Experience in community health settings and familiarity with Federally Qualified Health Center (FQHC) guidelines.
• A background in working with underserved populations, enhancing our commitment to equitable healthcare access.

The salary range for this position is competitive, ranging from USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
