Job description

Remote Mds Contractor Opportunity – Wisconsin

We are seeking an experienced MDS professional to join our team as a Remote MDS Contractor. This is an excellent opportunity for an experienced MDS nurse who is highly organized, knowledgeable in the RAI process, and comfortable working independently in a remote environment.

What You'll Do:
  • Complete and coordinate MDS assessments remotely
  • Support accurate, timely completion of MDS/RAI requirements
  • Collaborate with facility teams to gather necessary clinical documentation
  • Review medical records and ensure appropriate supporting documentation
  • Identify discrepancies and follow up with facility staff as needed
  • Assist with care planning and regulatory compliance
  • Maintain accurate documentation and meet established deadlines
What We're Looking For:
  • Active Wisconsin RN or LPN license
  • Strong MDS experience in skilled nursing/post-acute care
  • ECS experience required
  • Solid knowledge of the RAI/MDS process and CMS requirements
  • Strong clinical documentation and organizational skills
  • Ability to work independently and meet deadlines
  • Excellent communication and follow-up skills
  • Comfortable working with electronic medical records and remote technology
