Job description

Physical Therapist (PT), Occupational Therapist (OT), Speech Language Pathologist (SLP), Physical Therapist Assistant (PTA), Certified Occupational Therapy Assistant (COTA) needed for Full-time, Part-time, Weekend Contractor (1099) at a Skilled Nursing Facility in Bronx (10469) and Queens (Whitestone area 11357).

Candidate must have a valid NY license, has EMR experience (NetHealth, Sigma or PCC), be able to provide skilled rehabilitation services in a Skilled Nursing Facility (SNF) setting.

General duties and responsibilities:

  • Provide appropriate and timely evaluation of patients’ rehabilitative needs (PT, OT, SLP)
  • Develop therapy care plans using a variety of treatment techniques to address functional impairment and deficits
  • Provide quality, personalized and evidence-based care, and proven interventions
  • Motivate patients during treatment to help them participate and function optimally
  • Monitor, re-assess effectiveness, and communicate progress and potential barriers to therapy
  • Provide timely and appropriate documentation of skilled care provided

Job Requirements:

  • Valid NY State License
  • CPR Certification (Preferred)
  • Medical/Physical
  • Good organization and time-management skills
