Position Summary
The Referral Coordinator is responsible for coordinating, processing, and monitoring patient referrals to ensure timely access to specialty care and diagnostic services. This role serves as a liaison between patients, providers, insurance companies, and external healthcare organizations to facilitate seamless care coordination while delivering an exceptional patient experience.
The Referral Coordinator ensures referrals are processed accurately, authorizations are obtained when required, appointments are coordinated efficiently, and documentation is maintained in Epic in accordance with organizational, payer, and regulatory requirements.
Essential Duties and Responsibilities
Referral Management
  • Receive, review, and process incoming and outgoing referrals.
  • Coordinate specialty appointments for patients.
  • Monitor Epic referral work queues daily.
  • Ensure referrals contain complete clinical documentation.
  • Verify referral requirements according to payer guidelines.
  • Obtain and document prior authorizations as required.
  • Track referral status through completion.
  • Monitor referral aging and resolve delays promptly.
