Job description

The Transition Specialist for Indiana's Department of Correction: Division of Youth Services provides oversight to all transition-related practices in the State's juvenile correctional facilities.
Job Responsibilities may include, but not limited to:
• Facilitate the implementation of a life skills curriculum, titled Next Steps.
• Utilize the Transition Coordinator Handbook in an effort to standardize the transition practices.
• Evaluate the five "Transition Consultations" to be completed by every student under the guidance of an assigned teacher.
• Participate on a weekly basis in Administrative Release Committees (ARC) in each facility to confirm and reinforce the student's reentry plans.
• Do the Follow Up Phone Calls to released youth at 1, 3, 6, 9, and 12 months.
• The Transition Coordinators will represent the Indiana Department of Correction- Division of Youth Services at many conferences and meetings.
• The Transition Coordinators will assist in the development of a partnership with Indiana's Department of Workforce Development, HIRE, and Indiana University's HOPE Mentoring.
• The Transition Coordinators will assist in the development of the individualized Education Learning Plan for each youth upon entry at the facility.
• Work with state, county and private agencies to provide re-entry servicers for youth transitioning back into the community.
• Assist students with obtaining social security cards, state identification, driver's license, birth certificates, and additional documentation.
• Assist students with transitioning back to public schools
• Assist students with employment, apprenticeship programs, and military service information
