Position: Pendleton Transition Coordinator

- Position is on-site, Monday thru Friday.
- This position requires the ability to successfully pass CAI and DOC background checks including a drug screen, TB test, DCS check and fingerprinting.
- Position is located in a juvenile correctional facility and may be required to supervise students in the classroom in the absence of the classroom teacher.
- The Transition Specialist for Indiana's Department of Correction: Division of Youth Services provides oversight to all transition-related practices in the State's juvenile correctional facilities.
Job Responsibilities may include, but not limited to:
- Facilitate the implementation of a life skills curriculum, titled Next Steps.
- Utilize the Transition Coordinator Handbook in an effort to standardize the transition practices.
- Evaluate the five "Transition Consultations" to be completed by every student under the guidance of an assigned teacher.
- Participate on a weekly basis in Administrative Release Committees (ARC) in each facility to confirm and reinforce the student's reentry plans.
- Do the Follow Up Phone Calls to released youth at 1, 3, 6, 9, and 12 months.
- The Transition Coordinators will represent the Indiana Department of Correction- Division of Youth Services at many conferences and meetings.
- The Transition Coordinators will assist in the development of a partnership with Indiana's Department of Workforce Development, HIRE, and Indiana University’s HOPE Mentoring.
- The Transition Coordinators will assist in the development of the individualized Education Learning Plan for each youth upon entry at the facility.
- Work with state, county and private agencies to provide re-entry servicers for youth transitioning back into the community.
- Assist students with obtaining social security cards, state identification, driver’s license, birth certificates, and additional documentation.
- Assist students with transitioning back to public schools
- Assist students with employment, apprenticeship programs, and military service information
Requires skills:
- Ability to communicate effectively in a team environment.
- Experience with training, coaching or guiding youth.
- Experience collaborating and creating initiatives.
- Experience as a spokesperson at events.
- Experience with building relationships and partnerships to leverage resources and services.
