Recovery Coordinator

the work schedule will be (3) 12-hour shifts and then 2 days off alternating over a 6 week schedule.
About the Role
The Recovery Coordinator - Adult provides outreach, information-referral, case management, and a full range of mental health and rehabilitation services for clients. This includes counseling, crisis intervention, skill training, and coordinating and monitoring assessment, treatment planning, and follow-up for each client. Level II Recovery Coordinators may act as lead staff, providing supervision and training to other recovery staff and volunteers. This role requires the use of a personal vehicle for client transport and active engagement with clients in home and community settings.
Responsibilities
โข Conduct intake and comprehensive evaluation of each client's life situation, needs, and challenges
โข Develop a detailed plan of care in collaboration with clients
โข Assist clients in implementing their plan of care through community resource linkage and ongoing support
โข Monitor the effectiveness of support services, including client and provider participation
โข Provide professional support through client visits, consultation, guidance, and temporary interventions as needed
โข Transport clients to accommodate community relationships (personal vehicle required)
โข Maintain daily records, service progress notes, and billing forms per company and governmental standards (Short Doyle/Medi-Cal compliance)
โข Conduct outreach and provide information/referral services to persons not requiring case management
โข Maintain HIPAA compliance, ethical standards, and professional boundaries
โข Develop and maintain professional working relationships with agencies and service providers
โข Participate in management, staff, and client meetings, as well as training activities
โข Meet monthly productivity requirement of 100 hours
โข Perform other duties as assigned to support the program and company
Required Qualifications
โข AA, AS, or BA degree in human services, psychology, or related field, plus one year of related experience OR two years' equivalent experience
โข Valid California driver's license and auto insurance
โข Current CPR certification
โข Minimum one year prior case management or psychiatric treatment experience
โข Excellent prioritization, organizational, interpersonal, and communication skills
โข Intermediate PC skills
โข Related experience (preferred): coaching, scouts, teacher's aide, or CPS aide
โข Ability to complete six months of closely monitored training
Preferred Qualifications
โข Knowledge of rehabilitation models and mental health case management
