POSITION PURPOSE

  • Provide subject matter expertise in executive-level assessment of the current Capacity Management Command Center workflows, governance and structure for Transfer Center and Patient Placement functions. This assessment will evaluate existing practices, identifying operational and structural gaps, and provide high-level recommendations aligned with industry best practices to support the transition on to a centralized, system-wide Patient Logistics (PLC) model. Support accountable leaders for implementation and change management of logistics process.

  • Contingent position, 0.0 FTE, weekly hours not to exceed 20 hours.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Perform a comprehensive assessment of the current Capacity Management Command Center environment-including Transfer Center and Patient Placement infrastructure-evaluating operational workflows, role clarity, standard work and governance for the Capacity Management Command Center in Ann Arbor, MI (Southeast Michigan Region) as well as assessment of the individual facilities in the West Michigan Region (Shelby, Muskegon, Grand Rapids, Grand Haven) within Trinity Health Michigan

  • Facilitate structured stakeholder engagement with executive and operational leaders to gather insights, assess current-state capabilities and identify future-state opportunities, for alignment and organizational buy-in across the enterprise

  • Assess the current state and conduct a gap analysis, as well as deliver strategic recommendations to enable the implementation of a centralized enterprise PLC model aligned with industry-leading best practices for Trinity Health Michigan

  • Review and leverage qualitative and quantitative data to assess the current Capacity Management Command Center infrastructure, including Epic configuration on for Transfer Center 1 and Patient Placement, reporting and analytics, performance metrics, and established KPI targets

  • Perform an onsite visit to the Capacity Management Command Center in Ann Arbor, MI at Trinity Health Michigan, a minimum of 3 days, and conduct onsite visits at individual facilities es in the West Michigan Region (Shelby, Muskegon, Grand Rapids, Grand Haven), for a minimum of 1 day for each site.

  • Conduct a targeted review of Transfer Center performance reports in the Capacity Management Command Center to identify opportunities to enhance revenue capture through

  • Provide overall recommendations to project leads and Steering Team on Governance Structure, Job Descriptions, Epic enhancements, Technology and space needs, standard work, and KPIs.

  • Participating in and supporting in Steering Team Meetings

  • Support implementation and PDCA during go-live

REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE

  • Education: Bachelor's degree in related field (i.e. Human Resources, Social Work, etc.) highly preferred. Experience in organizational development/effectiveness/human resources or social work required.

  • Experience: Minimum of one year's experience in HR consulting, counseling and/or social work in a medical facility and/or mental health program and demonstrated leadership ability required.
