Peak Performance Therapies is seeking a dedicated and compassionate Licensed Physical Therapist (PT) to join our dynamic team at our state-of-the-art Rehabilitation Center located in Bridgeport, Connecticut. Work Environment

Working onsite at our rehabilitation center in Bridgeport, CT, you will find yourself in a modern and supportive setting that prioritizes patient comfort and outcomes. Our facility is designed with patient functionality in mind, featuring advanced rehabilitation equipment and a welcoming atmosphere that enhances the healing journey. This role offers a consistent Monday to Friday schedule from 2 PM to 8 PM, allowing you to maintain a balanced work-life dynamic.

Key Responsibilities
  • The Physical Therapist will manage the complete therapeutic care process for an assigned group of patients in collaboration with physicians and the care team.
  • Conduct thorough patient assessments to develop and implement individualized care plans that promote goal achievement and positive patient engagement.
  • Provide direct, hands-on physical therapy interventions, ensuring alignment with established legal and ethical standards in practice.
  • Evaluate and track patient progress regularly, adjusting treatment protocols as necessary to optimize rehabilitation outcomes.
  • Deliver therapeutic care within the patientโ€™s home, applying procedures that support consistent improvement in condition and functionality, based on the physician's treatment plan.
  • Instruct patients, families, and caregivers on effective use of therapeutic techniques, exercises, and devices to facilitate ongoing recovery efforts outside of therapy sessions.
  • Manage caseload priorities, determine staffing needs, and coordinate efficient patient care assignments to maintain workflow in the rehabilitation center.
Patient Population / Client Focus

Our rehabilitation center serves a diverse population, including adults recovering from surgeries, injuries, and chronic conditions, as well as pediatric patients needing specialized therapy. As a Physical Therapist, you will engage with patients at various stages of recovery, tailoring interventions to their unique needs and goals. Your role is critical in enhancing functional independence and providing educated support to families through the healing process.

Qualifications
  • A valid and active license to practice physical therapy in Connecticut.
  • A Doctor of Physical Therapy (DPT) degree from an accredited institution.
  • Basic Cardiac Life Support (BCLS) certification required within 30 days of employment.
  • New graduates are welcome to apply; no prior experience is required.
Preferred Experience

While we encourage new graduates to apply, candidates with previous experience in a rehabilitation setting or exposure to orthopedic therapy practices will be viewed favorably.
