This role is an onsite position, primarily focusing on delivering home care to our patients. You will be working within the familiar surroundings of patients' homes, enabling a comfortable and supportive environment for rehabilitation. • Deliver high-quality, patient-centered physical therapy services based on physician guidelines, tailoring treatment plans to meet the specific needs of each patient.
• Conduct comprehensive patient assessments to evaluate functional abilities, and implement targeted rehabilitation plans that focus on individual goal achievement.
• Administer therapeutic treatments in the home setting, utilizing advanced techniques to enhance recovery and improve patient outcomes.
• Instruct patients and their families in therapeutic exercises and the use of adaptive devices, ensuring they understand how to implement the care plan outside of therapy sessions.
• Maintain accurate and up-to-date patient records and documentation, coordinating with the broader healthcare team to provide seamless care.
• Prioritize and manage caseload effectively, ensuring that each patient receives adequate attention and treatment according to their specific conditions.

As a Physical Therapist at Peak Performance PT, you will have the opportunity to work with a diverse patient population that spans across all age groups. • Must hold a Doctor of Physical Therapy (DPT) degree from an accredited program.
• A valid Physical Therapy license in California is mandatory.
• Basic Cardiac Life Support (BCLS) certification is required to be obtained within 30 days of employment.

• While no prior experience is required, candidates with experience in home health settings will be given preference.
• Proficiency in using therapeutic modalities and a solid understanding of rehabilitation principles are advantageous.
• Experience in collaborating with multidisciplinary teams is valuable for this role.

The position offers a competitive salary ranging from USD 100,000 to 120,000 annually, complemented by a comprehensive benefits package.
