Key Responsibilities

As a Physical Therapist at Peak Performance PT, your day-to-day activities will encompass:

• Providing high-quality, patient-centered therapeutic care by managing the comprehensive treatment process for a designated group of patients in collaboration with physicians and multidisciplinary care teams.
• Conducting thorough patient assessments to develop and implement individualized care plans that focus on specific goals, enhancing patient engagement for improved outcomes.
• Delivering or directing substantial portions of physical therapy to facilitate rehabilitation and promote functional mobility, ensuring a positive continuity of care.
• Evaluating and documenting therapeutic/rehabilitative needs and participating in the holistic care plan development, considering each patient’s unique circumstances.
• Administering skilled therapeutic interventions at patients’ residences, using targeted procedural techniques aligned with established care plans.
• Educating patients, families, and caregivers in effective therapy techniques, exercises, and the use of therapeutic appliances or devices to promote independent practice outside of therapy sessions.
• Managing caseload priorities and coordinating daily staffing as well as hand-off processes for seamless care delivery.

Patient Population / Client Focus

This role primarily focuses on providing care to adult and pediatric patients recovering from surgeries, injuries, and chronic conditions. You will have the opportunity to work with individuals at different stages of recovery, enhancing their mobility and quality of life. By implementing evidence-based therapeutic interventions, you will play a crucial role in guiding patients through their rehabilitation journeys, adapting approaches to meet diverse needs within home care.

Qualifications

The ideal candidate will possess the following qualifications:

• A current license in Physical Therapy with the ability to practice in California.
• A Doctor of Physical Therapy (DPT) degree from an accredited institution.
• Basic Cardiac Life Support (BCLS) certification, or a willingness to obtain certification within your first 30 days of employment.
• New graduates and entry-level applicants are encouraged to apply.

Preferred Experience

While no prior experience is required, candidates with experience in outpatient physical therapy settings, knowledge of home care practices, or familiarity with a variety of therapeutic techniques will be viewed favorably. Experience working with interdisciplinary teams is also an asset in this collaborative environment.
