Job Summary:
We are seeking a highly motivated and compassionate Social Worker to join our team. As a Social Worker at AdviniaCare Summit Commons, you will play a vital role in assessing residents' needs, developing care plans, and facilitating discharge planning.

Responsibilities:

  • Assist in assessments, including Admission Assessments, Trauma assessments, and other evaluations as needed

  • Attend Care plan meetings and contribute to the development of individualized care plans

  • Assist in discharge planning, ensuring a smooth transition for our residents

  • Collaborate with the Director of Social Services to address resident needs and concerns

  • Support residents and families with emotional and psychological challenges

  • Maintain accurate and detailed records of assessments, care plans, and resident interactions
