Job Title: Social Work AssistantDepartment: LTC Social Work
Reports to: Director of Social Work
Looking for meaningful, heart-centered work that fits your schedule? Join our Social Work team as a Social Work Assistant and play a key role in supporting residents and families during important moments in their lives.
This role is perfect for someone who is compassionate, organized, and energized by building relationships. You'll work closely with residents, families, and an interdisciplinary care team to help ensure every resident feels heard, supported, and cared for.
What You'll Do:
  • Help support residents' social, emotional, and spiritual well-being
  • Assist with admissions and discharges, helping residents and families navigate transitions smoothly
  • Provide emotional support and crisis assistance when it matters most
  • Act as a connector between residents, families, care teams, and community resources
  • Participate in care planning meetings and collaborate with a supportive interdisciplinary team
  • Assist with documentation, including social services sections of required records
  • Build and maintain relationships with local and state community agencies
