WORK SCHEDULE: 1-3 days/week

FLSA CLASSIFICATION: Part-Time, Non-Exempt

COMPENSATION: The compensation range for this position is $35,000-$55,000, with the opportunity to earn additional income through a variable compensation (commission-based) structure, offering higher earning potential based on performance.

We are a multi-specialty healthcare organization dedicated to improving care for residents in skilled nursing facilities. Our clinicians provide essential medical and behavioral health services that support facility care teams and ensure patients receive consistent, high-quality care.ย 

Founded in 2018 and headquartered in Chicago, we partner with skilled nursing facilities across the Midwest and East Coast. Our providers deliver integrated clinical services including internal medicine, psychiatry, psychotherapy, and podiatry, helping facilities meet the complex medical and behavioral health needs of their residents.ย 

Position Summary:

Vitae Healthย is seeking both full-time and part-time Psychotherapists to provide mental health services to residents in a skilled nursing facility. The Psychotherapist will assess, diagnose, and treat emotional and behavioral conditions, supporting residents' overall well-being and adjustment to medical and life changes.ย 

Key Responsibilities:

  • Conduct comprehensive psychosocial assessments for residents upon admission and as neededย 
  • Develop, implement, andย monitorย individualized treatment plansย 
  • Provide individual, group, and/or family therapy sessionsย 
  • Address issues such as depression, anxiety, grief, cognitive decline, and adjustment to illness or disabilityย 
  • Support residents coping with chronic illness, pain, loss of independence, and end-of-life concernsย 
  • Collaborate with physicians, nursing staff, social workers, and rehabilitation teams to coordinate careย 
  • Participate in care plan meetings and interdisciplinary team discussionsย 
  • Maintainย accurateย andย timelyย clinical documentation in compliance with facility, state, and federal regulationsย 
  • Provide crisis intervention and behavioral management strategies when neededย 
  • Educate staff and families on mental health conditions and effective communication strategiesย 
  • Ensure compliance with HIPAA and ethical standards of practiceย 

Requirements

Qualificationsย 

  • Master'sย degree in Psychology, Counseling, Social Work, or related fieldย 
  • Active and unrestricted license inย respective stateย (e.g., LCSW, LPC, LMFT, or Licensed Psychologist)ย 
  • 2+ย years of clinical experience preferred, ideally in geriatric, long-term care, or medical settingsย 
  • Strong clinical assessment, diagnostic, and treatment planning skillsย 
  • Knowledge of mental health conditions common in SNF populations (e.g., depression, anxiety, dementia, adjustment disorders)ย 
  • Proficiencyย in evidence-based therapeutic approaches (e.g., CBT, trauma-informed care, grief counseling)ย 
  • Ability to work collaboratively within an interdisciplinary teamย 
  • Excellent communication and interpersonal skills with residents, families, and staffย 
  • Strong organizational, time management, and documentation skillsย 
  • Familiarity with electronic health records (EHR) systems preferredย
