Job description

JOB TITLE: Home Health RN Case Manager & Recertification Nurse (RNCM)
  • This position will be hired as PRN or Part-Time only.
    • Additional Part-Time ihours are also available with a 2-hour travel radius to "Secondary" locations listed below.
    • Full-Time may be available in the future but not at this time and is not guaranteed.
  • This is a regional leadership role within our agency, requiring travel as needed throughout Dayton and Metro-Dayton areas where clients and caregivers reside.
    • This role works closely with and reports directly to our SW OH Regional Program Administrator (local) and/or Director of Clinical Administration & Operations (corporate).
    • For those pursuing career growth: a defined path for advancement to Assistant Program Administrator, Program Administrator, Clinical Administrator, Director of Nursing (DON or Asst. DON), Business Administrator, QA / Compliance leadership, and other positions will be supported.
  • Work consists of: case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services, as well as new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall.
    • Requires back-up and occasional Skilled Nursing visits and services including med sets, dressing changes, etc.
  • At least 1 year of experience is required in: Case Management, Case Admissions and Discharges, wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non-OASIS assessments, and excellent documentation and field staff leadership.
  • At least 1 year of paid experience is required at/with a home health agency or in-home care provider (or equivalent); 2-5 years' experience is preferred.
  • Desirable but not required:
    • Nurse Supervision experience
    • People management
    • Peer/staff education and development
    • Success or willingness to build relationships with client payors, case mgr / entity relationships (i.e., AAA, DoDD County Board, VA, etc.) in SW Ohio and our current + prospective employees in these communities.

SERVICE AREA: Dayton / Dayton Metro and surrounding areas of up to 60-minute radius in Southwest Ohio
  • Primary / Centralized service areas include:
    • Dayton / Dayton Metro area
    • Middletown, Springfield, Xenia, Piqua, Urbana and surrounding areas
  • Additional case/hours are available for those willing to travel up to 2-hour travel (not required), to clients in/around:
    • Washington Court House, Wilmington, Salem Township, London, Circleville
    • Cincinnati / Cincinnati Metro area, Loveland, Lebanon, Hamilton
    • Columbus & immediate Columbus Metro area, Circleville
  • Qualified Applicants may reside in or near any part of the service areas so long as travel requirements are fulfilled.
  • NO overnight travel required.
  • NO Weekends required.

EMPLOYMENT & SCHEDULE TYPE: Long-term employment for PRN or Part-Time RNCM/Recert Nurse
  • This role requires weekday availability (Monday - Friday) between 8am - 5pm, with occasional/rare evening availability (scheduled in advance), and requires consistent, reliable availability during your selected Schedule Type.

    1. PRN (Ultra-Flexible): Maintain complete control over your calendar. You just need to accept an average of at least 1 patient visit per week over the course of the month to keep your active status.
    2. Part-Time (Consistent Hours & Earnings): Lock in a stable paycheck. You choose a set number of hours you want to work each week (Monday-Friday, 8 AM-5 PM). We fill those hours with patient visits based on company needs. The exact days and times you work may change week to week, but your income is consistent.
    3. Part-Time (Fixed Availability & Variable Earnings): Lock in a stable calendar. You define the exact days and times you are available every week. We fill that window with as many client visits as possible. Your hours may fluctuate, but your work times never change (and personal time or other job commitments are secure).
    4. Part-Time (Block Scheduling & Variable Earnings): Be available to work dedicated morning or afternoon "blocks" (e.g., 8 AM-12 PM or 1 PM-5 PM) on selected days. We fill that window with as many client visits as possible. This allows you to easily plan around childcare or school schedules. Your hours may fluctuate, but your work times never change (and personal time or other job commitments are secure).
  • PRN and Part-Time Nurses are paid Hourly, via weekly direct deposit on Fridays.
  • Round-Trip/Door-to-Door mileage is reimbursed.
  • Requires travel throughout pre-determined service area (minimum 1-hour travel radius); Willingness to travel up to approx. 2-hour daily radius) highly desirable.
  • Contractors will also be considered.
  • No On-Call Shifts / No Weekends required.
    • Weekend services are optional when available but are not required.

Compensation:

Job Summary As an RN Recertification Nurse, you will be instrumental in coordinating, re-evaluating and confirming the medical necessity of continued home health care services for our patients, and providing patient care and ensuring optimal outcomes.
  • With core skills in managed care, patient assessments and compliance, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.
  • You will submit updated documentation of the patient's condition and ongoing care needs timely and within compliance of all state, federal, and agency guidelines.
  • You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.

Responsibilities/Requirements of an RN Case Manager / Clinical Supervisor include:
  • RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • RN to Document/Communicate/Coordinate patient status/change in condition to care team.
  • RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
  • Minimum of one (1) years of nursing supervisory experience. Home care experience preferred.

Reports to: Regional Program Administrator, or Clinical Administrator/Director of Nursing
Qualifications
  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • Minimum of 1 year of experience in home health agency or equivalent setting.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
  • Experience with OASIS and NON-OASIS assessments is also required.
  • Strong background in nursing, patient care, and confidentiality.
  • Exceptional on-time and attendance record, and consistent availability aligned to your chosen schedule type (PT/FT/Hybrid)
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
  • Familiarity with HIPAA regulations and medical terminology.
  • Willingness and ability to perform required physical demands (see below)

  • 18yo + active Driver's License / State ID + US Work Authorization
  • Reliable transportation to all scheduled visits, within full reach of stated travel radius and assigned clients (up to at least 2 hours is required).
