  • Skilled Visits to patients based on their needs.
  • Daily patient interaction, ensuring comfort and support.
  • Education on health conditions including, but not limited to, CHF, COPD, Diabetes.
  • Wound care and assessments.
  • Medication management and administration.
  • Change dressings, bandages, and other contraptions as prescribed.
  • Documentation of patientโ€™s condition, reporting any significant changes to the physician.
  • Collaboration with other healthcare professionals, including Physical, Speech, and Occupational Therapists.

Requirements

  • Active RN Nursing License in the state.
