Chat and Change Mental Health is seeking a knowledgeable Licensed Clinical Social Worker (LCSW) to join our dedicated team in Port Saint Lucie, Florida. This full-time remote position offers the opportunity to provide essential mental health services to individuals facing various psychological challenges. This position allows for a flexible remote work arrangement, while dedicating time to connect with patients through various telehealth modalities. You will be working within a robust virtual team, comprising mental health professionals committed to team collaboration, regular supervision, and peer support. Our operational hours are Monday through Friday, from 9:00 AM to 5:00 PM, creating a structured environment that enhances work-life balance.

• Deliver high-quality psychotherapy services to patients suffering from mental health conditions such as anxiety, depression, and post-traumatic stress disorder (PTSD).
• Conduct comprehensive mental health assessments, determining patients' emotional, behavioral, and social functioning to inform effective intervention strategies.
• Develop, implement, and adjust personalized treatment plans that are evidence-based, culturally sensitive, and tailored to each patient's unique needs.
• Collaborate with physicians, psychiatrists, and other healthcare providers to ensure an integrated approach to patient care and create a supportive network for clients.
• Monitor patients’ progress and modify treatment plans as needed based on patient feedback and healthcare team input.
• Provide education and resources to patients and their families regarding mental health conditions, treatment options, and community support services available.
• Uphold ethical and professional standards in all practices, ensuring confidentiality and obtaining patient consent for treatment.

Your work will primarily focus on adults facing a variety of mental health challenges, which may originate from trauma, life changes, and chronic illnesses. As part of a hospice center, you will have a unique opportunity to provide services to individuals dealing with end-of-life issues, requiring sensitivity and expertise in coping mechanisms and grief counseling.

• A Master's degree in Social Work from an accredited institution is required.
• Current state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of 2 years of experience providing mental health services, demonstrating competency in delivering psychotherapy to diverse populations.
• Exceptional communication and interpersonal skills to develop trust and rapport with patients and collaborate effectively with healthcare professionals.
• Strong clinical aptitude, particularly in conducting assessments and formulating evidence-based treatment plans.
• Experience with electronic health records (EHRs) and proficiency in utilizing healthcare software to document patient care and coordinate treatment.
• Knowledge of various mental health disorders, treatments, and the complex dynamics patients face in their recovery journeys.

• Experience working within hospice or palliative care settings, dealing with terminal illness and the emotional aspects of end-of-life care.
• Familiarity with community resources and support systems to enhance patient care and connectivity beyond therapy sessions.

The salary range for this position is competitive, ranging from $90,000 to $105,000 per year, commensurate with experience and qualifications. We offer a comprehensive benefits package that includes:
