JOPLIN GARDENS, located in Joplin, Missouri is currently seeking an MDS Coordinator to join our team.

MDS Coordinator Duties and Responsibilities:

  • Completing accurate assessments, MDS & care plans as assigned
  • Initiating care plans and supporting activities as assigned
  • Creating and distributing monthly care plan calendars in a timely fashion
  • Maintaining & updating all care plans and assessments as required
  • Monitoring & auditing clinical records, ensuring accuracy & timeliness
  • Informing DON of persistent issues related to non-compliant documentation
  • Protecting the confidentiality of Resident & Facility information at all times
  • Participates in various meetings and committees: Medicare, QAPI, Weight Management etc.

MDS Coordinator Qualifications and Skills:

  • Valid RN or LPN License
  • Knowledge of skilled nursing facilities survey process and regulations would be helpful.
  • Must have prior MDS Experience
  • Long Term Care Experience Required
  • Must be able to work 40 hours a week
  • Must be highly organized, professional & motivated
  • Should have solid computer skills
